Medical history Female, 33 years old. On the 30th postpartum day, she had complaints of fever, pain, swelling in the left breast. On physical examination: hyperemic, hardened and painful periareolar region (Image 1). After clinical diagnosis of puerperal mastitis, amoxicillin with clavulanate, local ice were prescribed and breast milking at the end of breastfeeding was started. She used the antibiotic for 2 days, but progressed with worsening of the clinical picture and the appearance of periareolar fluctuations, at “10 o’clock” (Image 2).
