Medical history A 12-year-old female attends to the Center of Information and Toxicology Assistance in Belo Horizonte (CIAT-BH) complaining of severe pain in the left hand after catching a ball that have fallen into the woods, nearby her house, and stated she did not know what caused the injury. Refers burning pain, affecting the left upper limb as a whole. At the physical exam, presented HR:110bpm, RR:30cpm, Sat02:98% and a skin rash as in the attached image, with no further findings. Image 1: Dorsal face of the left hand Question:Based on the clinical picture, the most correct workup would be: Give her at least 2 doses of scorpion antivenom, since she is a patient with increased risk for complications Do a scrapping of the site under anesthesia, manage the pain and look for indications to test the coagulation profile Give at least 4 doses of Bothrops sp. antivenom given the classical presentation Observe the patient for 24h, as a precautious measure, since spider accidents are not usually lethal Time is Up!
